Hepatitis B Knowledge and Vaccination Status among Ethnic Kashmiri Population: A Community Based Cross-Sectional Study

AbstractHepatitis B virus (HBV) infection is one of the major global public health problems with nearly 2 billion people infected worldwide. So this present study was planned to have information about socio-demographic characteristics of study subjects, level of knowledge about Hepatitis B and vaccination among adults. A cross-sectional, community-based study about hepatitis B knowledge and vaccination status among ethnic 1300 Kashmiri population aged 18 years and above was conducted. The study area was block Hazratbal of district Srinagar. There were 970 (74.6%) females and 330 (25.4%) males. Majority of our participants (54.7%) were in the age group of 21-40 years. Most of the participants were from urban areas(68%), currently married(66.8%), illiterate(64.8), members of joint family(59%) and belonging to socio-economic class II(67.2%). Regarding knowledge, only 10.2% subjects had heard of Hepatitis B before this study. Among them, 50 (37.6%) participants were aware of the modes of transmission of this disease. About the Hepatitis B vaccination, only 26 (2%) participants out of 1300 had ever received the vaccine. Keeping in view, the low level of knowledge about Hepatitis B in the general population, there is a need to organize health education campaigns targeting both health care workers as well as public, so that they adopt all possible measures to prevent the spread of this fatal infection.

Descriptive analysis of Traumatic Cataract Cases with Special Reference to its Surgical Outcomes

Abstract— Traumatic cataract is the commonest complication causing diminution of the vision following any type of ocular injury. It causes serious visual disability in working population So this study was aimed to analyse the post traumatic cataract cases and to observed its surgical outcomes. This study was conducted on 47 post traumatic cataract cases attended at Opthalmology department of SMS Medical college, Jaipur. Present study observed that males predominate over females in traumatic cataract with M:F ratio 2.7 and majority (59%) of cases were below 20 years of age. The most common mode of injury was wooden stick in 40.43% cases followed by stone in 23.40%. 85.5% patients had pre operative visual acuity below hand movement in affected eye. In most of the cases IOL was implanted successfully and only one case left aphakic due to inadequate zonular support. Intraoperative posterior capsular rent occured in 1 case, and vitreous prolapse in 2 cases. Postoperative complications were striate keratopathy, AC reaction, residual cortical matter and hyphema. In this study final visual outcome in affected eye were as follows-44.68% patients -20/20 – 20/30, 38.30% patients – 20/40 -20/60. 82.98% patients were gain satisfactory final visual outcome. 17.02% patients have visual acuity below 20/80, due to the co-morbidity.10.64% patients have posterior capsular opacification, 6.38% patients have vitreous hemorrhage and one patient have aphakia with vitreous hemorrhage. These all patients were referred for further management. This study concluded that final visual outcome in traumatic cataract patients in closed globe injuries is clearly related to extent of damage to anterior and posterior segment. PCIOL implantation is the best way of rehabilitation of traumatic cataract cases. Further with better surgical techniques and instrumentation, minimal handling of tissues during surgery and good pre-operative and post-operative control of inflammation better results can be achieved.

Anemia among Adolescent Girls and its socio-demographic Associates

AbstractAdolescents are the future resources for any country to progress and prosper. According to Census 2001, in India, adolescent constitute one-fifth of the total population. Adolescent AGs are one of the important segments of the population for they are the future mother whose nutritional status affects that of the newborn baby. Anemia being a major public health problem among adolescent girls, a study was conducted with the objective to determine the socio-demographic correlates of anemia among girls. Methodology: A cross-sectional study was conducted among 467 adolescent AGs in Ahmadabad city during May 2011 to august 2012 in Adolescent Friendly Health Services clinics. Results: Prevalence of anemia among adolescent girls was 85.9%. Highest prevalence was observed during mid-adolescence phase. As the age of girls increases, Hemoglobin tends to get on the lower side. A statistically significant association was observed between anemia and nutritional status. No association was observed between anemia and religion, birth order, type of -family, education of parents and occupation of parents. Conclusion: Association between anemia and under-nutrition has been reported earlier in numerous studies; however present study highlights the fact that it is not common to have anemia among girls who are over nourished or obese. Hence, special efforts should be made to address this issue among girls by doing dietary modifications.

AbstractTo strengthen the surveillance system in India, Integrated Disease Surveillance Program (IDSP) was launched in 2004. The frequent occurrence of epidemics even after the launching of the IDSP was an indication toward inadequacy of the system. The aim of the this study was to find out the IDSP disease pattern and load on a tertiary hospital. It was cross-sectional study carried out in hospitals attached to SMS medical College, Jaipur (Rajasthan) India. Weekly report of IDSP in 'P' Form was collected from SMS Medical College, Hospitals. Data related to IDSP diseases were gathered from these reports. These reports were analysed in percentage and proportion. It was observed in this study that among IDSP diseases most common was fever of unknown origin accounting total 93 (23.97%) cases followed by Acute Diarrheal including Ac. Gastroenteritis, Acute Respiratory Infection (ARI) Influenza like illness (ILI), Pneumonia, Malaria, Viral hepatitis etc. Distribution of various IDSP diseases were with significant variation in pediatric and adult population. Among pediatric population ADD was most common whereas in adult population ARIs were most common. Even after launching of more than a decade, a sizable burden of IDSP diseases is there at tertiary level hospital, who could be treated at peripheral health institutes like Sub centre and Primary health centre. So there is a strong need for IDSP disease and its toll free no awareness.

Non-alcoholic Fatty Liver Disease (NAFLD) and its association with metabolic syndrome and cardiovascular diseases

Abstract Non Alcoholic Fatty Liver Disease is also becoming public health impotance nowadays. So this study was aimed to determine the association of Non Alcoholic Fatty Liver Disease with metabolic syndrome and Cardio-Vascular disease along with assessment of degree of severity of NAFLD with respect to number of components of metabolic syndrome. This study includes a total of 222 subjects were enrolled as per the inclusion/exclusion criteria, out of which 110 cases who had NAFLD with hepatic steatosis on ultrasonography and 112 subjects who did not have NAFLD were considered control. These cases and controls were interrogated and investigated further. Observations were recorded and association of Non Alcoholic Fatty Liver Disease with metabolic syndrome and Cardio-Vascular disease along with assessment of degree of severity of NAFLD with respect to number of components of metabolic syndrome. Statistical methods used were unpaired student’s t-test for continuous variables, Fischer’s and chi-sq test for categorical variables using bivariate analysis by Graph Pad Instat Version 3.10. Risk was assessed in terms of Odd's Ratio. The patients with MS and NAFLD had a higher proportion of CVD compared with those who did not have NAFLD (29.1 vs 18.1 %). This study concludes that NAFLD is significantly associated with MS; most significant with WC, followed by TG and FBS and thus can be considered as hepatic component of MS. This needs more research with large multi-centric prospective studies to evaluate NAFLD as an independent risk factor for CVD.

Etiological pattern of Lymphadenopathies and Role of Fine Needle Aspiration Cytology (FNAC) in its Diagnosis

Abstract—Lymphadenopathy is one of the commonest presentation in inflammatory and neoplastic cases. Pathological diagnosis of enlarged lymph nodes is crucial in further management of patients. Fine Needle Aspiration cytology (FNAC) is quick and cost effective OPD procedure for establishing etiology of enlarged lymph nodes. This study was aimed to observed the pattern of lymphadenopathy as per FNAC and its diagnostic accuracy assuming histopathology as gold standard. This study was conducted on two hundred and thirty one consecutive enlarged lymph nodes attended for FNAC in a secondary care level Government Hospital, Gandhi Nagar, Jammu in a study period of two and a half years. Lymph nodes of these cases were aspirated and subjected to cytomorphological evaluation with Papanicolaou (PAP) and Giemsa stain. After that histopathological examination was done of excised biopsies. Then pattern of lymphadenopathy as per FNAC was observed and its diagnostic accuracy was found out assuming histopathology as gold standard. Maximum number of patients was in the age group of 21-30 years age group with male to female ratio 1.2:1. Out of 231 lymphadenopathy cases 4 remain inconclusisve whereas 200 (88.11%)cases were benign and 27 (11.89%) were malignant including 14 (6.1%) cases of metastatic tumors. Among benign cases, majority had non specific reactive lymphadenitis (42.29%) followed by tubercular lymphadenitis. And among malignant tumors, metastatic tumors (6.1%). were most common. Diagnostic accuracy of FNAC was observed as fairly good i.e. ranging from 100% to 83.3% in various type of lymphadenopathies. So it can be depicted that FNAC is very useful first line investigation in patients presenting with enlarged lymph nodes especially in secondary level health care hospitals/centers where advanced diagnostic modalities are not available. The suspicious cases can always be referred for further evaluation.

Bacteriological Profile of Burn Wound Infection in a Tertiary Care Hospital in North India with Special Reference to Methicillin Resistant Staphylococcus aureus

Abstract Methicillin-resistant Staphylococcus aureus (MRSA) poses a great risk to burn patients with potential to cause significant morbidity and mortality. This study aimed to find out the prevalence of MRSA and its susceptibility, in burn wound infection/colonization in a Tertiary Care Hospital in North India. A retrospective study was conducted among patients admitted in burn ward of our hospital, between January to December 2012. All the patients irrespective of age, sex, duration of hospital stay, percentage and degree of burn were included in our study. Wound swabs from 1294 patients hospitalized in burn ward were analysed for bacteriological examination. Swabs were inoculated on Blood agar, MacConkey agar and Brain heart infusion broth. Isolates were examined for colony characteristics, Gram staining and biochemical tests. Antimicrobial susceptibility testing was done by modified Stokes disc diffusion method. Detection of MRSA was done by cefoxitin (30mg) disc diffusion method. Among the Staphylococcus aureus (S.aureus) isolates, 56.7% (80/141) were found to be MRSA while 43.3% (61/141) were Methicillin Susceptible S.aureus (MSSA). All the MRSA isolates were resistant to penicillin, cephalexin and cefazolin. Resistance to erythromycin, clindamycin, ofloxacin, ciprofloxacin, gentamicin, amikacin, rifampicin, chloramphenicol was found to be 74%, 97.4%, 96%, 100%, 97.4%, 84.6%, 11.5%, 10.3%. All MRSA isolates were found to be sensitive to vancomycin and teicoplanin while 1.3% were resistant to linezolid. Although survival rates for burn patients have improved substantially over the years, nosocomial infections still remain a major challenge in burn care. This concludes that there is high prevalence of nosocomial infections specially the presence of multidrug resistant bacteria like Methicillin Resistant Staphylococcus aureus among burn patients suggest continuous surveillance of burn wound infections and development and stringent implementation of antibiotic policy.

Key-words: Methicillin Resistant Staphylococcus aureus (MRSA), Methicillin Susceptible Staphylococcus aureus (MSSA), Antimicrobial susceptibility testing, burn wound infection.

Association of Serum Lipid Profile and Chronic Periodontitis: A Case Control Study

AbstractPeriodontal disease is a destructive inflammatory disease inducing profound changes in the plasma concentrations of cytokines leading to a catabolic state characterized by altered lipid metabolism and hypertriglyceridemia. This study was conducted with the aim find out association of chronic periodontitis with serum lipid parameters. Study group consist of 30 cases of chronic periodontitis (case group) and control group consist of 30 healthy individuals. Age range was kept 25-60 years to avoid extreme ages. Periodontal parameters including Plaque Index, Gingival Index, Probing Depth and Clinical Attachment Level were recorded. Lipid profile comprising of total cholesterol, Triglycerides, HDL- Cholesterol and LDL was assessed and co related with periodontal parameters. This study confirms significantly higher levels of mean cholesterol, triglycerides and LDL in periodontitis group as compared to healthy group. Also, there is significant negative co relation of HDL with probing depth and clinical attachment loss. Association of hyperlipidemia and chronic periodontitis is evident in developed state of disease. With this study, this relation is confirmed based on Factor and Outcome.

Association of Zinc, Copper and Iron Levels with Birth Weight

AbstractThis study was aimed to find out relationship of maternal Zinc, Copper and Iron levels with birth weight. Epidemiological observations are that Low birth weight babies i.e. weighing less than 2,500 grams, are approximately 20 times more likely to die than heavier babies. LBWs are more common in developing than developed countries. This study was conducted on 100 new born infant and their mother. Mothers of just delivered babies were investigated for assessing serum zinc, iron and copper levels on atomic absorption spectrophotometer. Along with this cord blood samples of newborn delivered by these identified women were investigated for zinc, iron and copper levels. Simultaneously birth weight of newborns delivered by these mothers were assessed. Association of maternal Zinc, Copper and Iron with birth weight of newborn was analyzed by unpaired student's’ test of significance. It was found in this study that maternal and newborn serum Zinc and Copper level were significantly lower and higher respectively in LBWs whereas serum Iron was not associated with birth weight of infants.

Peripheral Ossifying Fibroma with Superficial Bone Erosion: A Case Report

AbstractThis study was aimed to present a case report of a case of peripheral ossifying fibroma which is a rare case. This case was a 30 years non smoker male with the chief complaint of growth of gum tissue, moderately large in the mandibular posterior region. On intraoral examination, a peduncalated growth of 17 x 12 x 6 mm on marginal and attached gingiva with respect to tooth number 47 considerably hard in consistency and movable was seen. The lesion was erythmatous having a smooth non ulcerated surface. It was asymptomatic with no sign of pain. Intra oral periapical radiograph was taken which revealed slight erosion of crest of bone which was later confirmed during surgical excision. The possible reason of crestal bone erosion may be constant pressure of the growth. Differential diagnosis of irritation fibroma, pyogenic granuloma and peripheral giant cell granuloma was considered. However, clinical appearance and consistency was of a hard fibrous growth, which therefore led to a provisional diagnosis of peripheral ossifying fibroma or peripheral odontogenic fibroma.

Maternal factors of anemia in 3rd trimester of Pregnancy and its association with fetal outcome

Abstract—This study was aimed to find out the maternal factors and fetal outcomes associated with anemia in 3rd Trimester pregnancy. A hospital based cross sectional descriptive type of observational study was carried out in 15 to 49 years who had undergone delivery at SP Medical College Bikaner. Information about the demographic profile, ANC factors and foetal outcome data were collected. To find out associating factors appropriate test of significances were used. The magnitude of anemia 91.3% (995/1090) was found high in third trimester of pregnancy. Caste, dietary – habit, Education, Occupation, Socio-economic status, ANC Visit, Iron and folic acid supplementation were associated (P<0.05) with anemia whereas maternal-age (year) Residence Religion Number of Children Type of family Inter-Pregnancy Interval (Months) were not associated (P>0.05) with anemia. Although IUGR, premature births and still births were observed respectively 2.56 times (95% CI: 0.615 to 10.697 ), 1.3 times (95% CI: 0.723 to 2.351 ) and 0.651 times (95% CI: 0.286 to 1.481 ) in anemic mothers but it was not found significant with anemia status. But significantly more (35.5%) low-birth weight babies were born to anemic mothers as compared to ( 14.7% )among non- anemic mothers i.e. 3.181 times (Odds ratio) higher LBW babies in anemic mothers. Anemia in pregnancy may be reduce by proper Iron and folic acid supplementation which can be improved through IEC and providing proper ANC services.

Spiritual Intelligence: A Road Map to Future Learning

Abstract Rational Intelligence helps us take logical decisions while emotional and spiritual intelligence can be blended together in a beautiful way. IQ and EQ have given a way to Spiritual Intelligence, which is considered as the base of all intelligences. It has been tried out through this study that how Spiritual Intelligence can guide a leader in the organisation to think beyond his/her own personal goals/identity. Authors have tried to conceptualise the Spiritual Intelligence / Quotient Model which enumerate the characteristics of a leader who is spiritually intelligent and also tries to co-relate the concept of Learning Organisational Culture with the extensive and wide use of Spiritual Intelligence in the organisations. Literature available on the subject has been reviewed extensively to arrive at the following concepts and to establish the correlation between Spiritual Intelligence and Learning Culture/ Habits of the Organisations. To test the above concepts, authors have chosen two Public Sector Undertakings, one is Bharat Petroleum, which has been a Fortune 500 Company and the other one is NTPC, which is a leading Navratna status and profit making Public Sector Organisation. It was observed that academically the respondents at all levels were unknown to this “Spiritual intelligence”, and they understood the concept as alternate to religion. In the view of literature the characteristics of spiritually intelligent leader should includes intrapersonal characteristics like self awareness, self realization, Self actualization, Self respect, Organizational awareness, Organizational actualization, Independence, Assertiveness, selfness and Altruistic Attitude along with other interpersonal characteristics, Stress management characteristics, Adaptability etc.

Abstract— Papillary Hidradenoma is a rare benign tumor of apocrine glands. Very limited number of case reports are available in the literature detailing the cytological features of papillary hidradenoma. Such a rare case specimen came in notice here at Pathology Department of SMS Medical College, Jaipur (Raj.) India. So, one such a rare case of papillary hidradenoma was explored with its cytological findings. A 30 year old female presented with a vulval cyst. Fluid from this cyst was sent for cytology with a clinical diagnosis of Bartholin cyst. The cytology suggested it to be a benign adnexal tumour by the presence of Biphasic pattern of cell arrangement which was further confirmed histologically. So whenever an middle aged female presents with a nodular lesion in the anogenital area, hidradenoma papilliferum should be kept in mind along with other conditions.
